I'm looking for help caring for a loved one. What services might be available?

Long term care refers to a broad range of services that are coordinated to meet the needs of those with chronic illnesses and disabilities. Services should be planned over a period of time because our loved one's needs and service requirements change. Long term care may be delivered in the home, in the community or in an institution.
People plan for retirement but often don't plan for long term care needs. Will you be able to remain in your home? What services are in your community? Can you afford the services?
There are agencies that offer in home assistance providing help with housekeeping, meal preparation and personal care. Home delivered meal programs may be able to provide a hot meal for a fee or a donation for those who can't get out. Stores will deliver groceries and medications.
Community based services are found in many communities and may offer Adult Day Service programs, congregate meal sites and activities. Support groups may be available for caregivers.
Institutional care may be needed for your loved one. This may involve a short stay in the hospital to address an acute care need or perhaps in-patient care in a behavioral health facility is needed. Assisted living or personal care may provide sufficient care for a loved one who needs room and board, supervision, and medication administration. For those who need 24-hour care and nursing supervision, a nursing home may be required. You may want to consider a continuing care facility or community where your loved one can move along the care continuum in one location. Many continuing care communities offer independent housing through nursing care.
